The MOST Common Cause of Dizziness - Benign Paroxysmal Positional Vertigo (BPPV) - Diagnosis & Treatment. Dr. Cliff Olson, Audiologist and founder of Applied Hearing Solutions in Phoenix Arizona, discusses the #1 cause of dizziness - BPPV. Website: 🤍 Clinic Website: 🤍 You know that feeling you get when you get off of a merry-go-round and you feel like you are still spinning? Well that is what it is like to have Benign Paroxysmal Positional Vertigo. BPPV causes dizziness following head movements or body position movements. Basically, if you have ever become dizzy by just rolling out of bed, you may have had BPPV at some point. BPPV is the most common cause of dizziness. Let's explore the description of BPPV. Benign means that it won't kill you. Paroxysmal means that the episodes are of brief duration. Positional means that it is triggered by movement, and Vertigo is the sensation of movement, such as spinning, when there is no head movement. BPPV occurs when crystals called otoconia, normally embedded in the utricle become dislodged and migrate out to one of the three fluid filled semicircular canals. These crystals DO NOT belong in the semicircular canals. As head positions change, cause additional stimulation of the sensory hair cells in the semicircular canals which sends false information to the brain which leads to the sensation of dizziness. BPPV is the most common cause of vertigo with 50% of individuals over the age of 70 experiencing this type of dizziness. Symptoms of BPPV may come and go, and you may feel symptom free for a period of time, as long as you refrain from significant head movement. It is important to know that hearing loss is not a symptom of BPPV. If you have hearing loss along with dizziness, this could indicate a serious medical condition and you should see your physician immediately. Diagnosis of BPPV requires structured positional movements along with the evaluation of nystagmus which are involuntary eye movements. If a nystagmus is seen and the appropriate semicircular canal is identified, then treatment can be administered. According to Dr. Richard Gans of the American Institute of Balance (🤍) treatments called Canalith Repositioning can be used to guide the dislodged crystals back into the Utricle. These repositioning techniques are extremely effective, but should only be done under the supervision and assistance of an individual specifically trained in the diagnosis and treatment of dizzy conditions. Attempting to diagnose and treat BPPV yourself can be dangerous considering that if you perform treatment incorrectly, you could make your condition worse. The good news is, diagnosis and treatment of BPPV is relatively straight forward when treated by a specially trained Physician or Audiologist. Just make sure to follow your Doctors post treatment instructions. So the next time you experience intense, brief spells of dizziness, when you change head or body position, make sure you contact your Audiologist or Physician to get your condition diagnosed as soon as possible.

What is Vertigo? Vertigo is a sense of rotation, rocking, or the world spinning, experienced even when someone is perfectly still. What is the Physiology of Balance? The human balance system works with our visual and skeletal systems to maintain orientation or balance. Visual signals are sent to the human brain about the body's position in relation to its surroundings. These signals are processed by the brain, and compared to information from the vestibular, visual and the skeletal systems. Our brain receives inputs from our eyes, muscles, joints and vestibular system in our ears, then it process these nerve impulses so that it can give the person a correct perception of the environment. Our ears contain special parts like the saccule, utricle and three semicircular canals which are all part of the vestibular apparatus. The purpose of vestibular apparatus is to provide the brain the correct perception of equilibrium (balance), spatial orientation and motion. The parts that are responsible for vertical orientation are the saccule and utricle. The rotational movement is detected by the semicircular canals that contain fluid known as endolymph. The endolymph moves and triggers the sensory receptors within the canal which sends nerve impulses to the brain regarding the movement. Impulses from both canals of the side of the head would normally send symmetrical impulses in order for the brain to interpret the impulses properly. Types of Vertigo? There are two possible causes of vertigo: peripheral vertigo and central vertigo Peripheral Vertigo: PERIPHERAL VERTIGO REFERS TO THE DISORDERS THAT INVOLVE DISTURBANCES IN THE INNER EAR. The common disorders that may affect the inner ear thereby causing the person to experience dizziness or vertigo include: Vestibular Neuritis – Inflammation of the vestibular nerve which is responsible for taking the nerve impulses from the semicircular canals in the inner to the brain. Labyrinthitis – Inflammation of the labyrinth which is also in the inner ear and most of the time may also involve the vestibular nerve. Both of these disorders are usually caused by a viral infection that may have reached the inner ear. Other causes include: Meniere’s Disease: This occurs due to increased fluid in inner Ear. Benign Paroxysmal Positional Vertigo or BPPV: This result from the movement of the otolith, which is a very small particle made of calcium inside the ear. The otolith organs detect the movement of your head and its relation to gravity. For some reason the otolith can be dislodged and move into the semicircular canals and affect the movement of the fluid inside thereby causing it to send irregular nerve impulses to the brain making the person get dizzy. How to treat BPPV?: The most common and effective treatment for BPPV is known as the canalith repositioning procedure. The procedure consists of several simple and slow maneuvers for positioning your head. The aim is to move the particles from the semicircular canal to the utricle where they don't pose a problem. Each position is held for around 30 seconds after which any symptoms or abnormal eye movements stop. #FreeMedicalEducation #Vertigo

DIZZINESS FROM THE NECK. (CERVICOGENIC DIZZINESS EXERCISES) If joints are jammed/restricted in your upper neck, or lower neck, this will throw out your body’s positional sense. (It can send information about your position that conflicts with your inner ear and eyes.) If you have a tight or stiff upper neck or headaches it more likely that your upper neck is the cause. If you have really tight shoulders, poor neck posture, or looking at the ceiling makes you feel dizzy, the lower neck is more likely to be what makes you dizzy. So, the first two exercises I give you in this video aim at loosening the joints and muscles in your neck. 2. DIZZINESS FROM EYE PROBLEMS (VOR OR GAZE STABILITY EXERCISES) If your eye muscles are weak, or the connection between your eyes and the balance system of your brain is weak, “gaze stability exercises” may be what you need. I show you how to test for this and how to correct it quickly. 3. DIZZINESS FROM THE INNER EAR (BPPV EXERCISES) Sometimes stray crystals can find their way into the semi-circular canals of your inner ear. The crystal’s movement can confuse the detectors within the canal causing something called BPPV. (Benign Paroxysmal Positional Vertigo.) If you get a real spinning sensation a few seconds after turning onto one side, when you’re lying in bed, it’s likely you have this problem. I show you how to do the Dix-Hallpike test for BPPV, to see if crystals in your inner ear are causing your spinning or vertigo sensation. The Epley Manoeuvre is probably the most famous treatment for BPPV, but research suggests that a much simpler technique—Brandt-Daroff vestibular training—is equally as effective (and less likely to cause distressing dizziness as you do it) so I show you how to do that. OTHER CAUSES OF DIZZINESS AND VERTIGO: If your dizziness continues, always get it checked out because there are more serious causes of dizziness and vertigo. • Meniere’s disease is one possibility and often comes with hearing loss, a blocked ear feeling, or a roaring tinnitus. • Stress can cause a floating sensation. • Blood pressure problems (high or low) can cause dizziness especially with a change of position like getting up from either sitting or lying down. • Medication side effects. • Other causes include stroke and cancer thought, so seek immediate medical attention if you also experience a severe headache, fever, difficult speaking, weakness in your arms or legs, double vision, blindness, loss of consciousness, numbness or tingling, or a fever with your dizziness.
